Abstract: Soft tissue sarcomas are rare mesenchymal tumours usually occur in the extremities or the retroperitoneum. Fibrosarcoma is diagnosed in about 10% of the cases of soft tissue sarcomas. These are usually managed with extensive local resection and post-operativeradiotherapy. Retroperitoneal sarcomas are prone to local recurrences while distant metastases are more common in extremity lesions. Our case presented to us with recurrent masses in the left leg and pain in the masses. The patient was operated thrice in a period of 6 years and local resection was done. The patient was given radiotherapy after the third surgery. The case was a rare third local recurrence within a short period of 6 years without any distant metastasis. Post –operative radiotherapy after the local excision is a must to prevent recurrences.

Abstract:Background: A variety of treatment modalities are available for fracture of distal radius. However, there is a lack of consensus on the preferred treatment in these cases. Aims: To compare the different methods for the treatment fracture of distal part of the radius based on the outcome at the end of treatment. Methods: Analysis of cases with fracture of distal radius treated at our centre over a period of seven years. A total of 301 patients were included in the study and end of treatment outcome was compared based on clinical appearance of the wrist at discharge, restoration of normal movements of the wrist joint and resumption of normal activities.

Abstract: Autogenous tooth transplantation, or autotransplantation, is the surgical movement of a tooth from one location to another in the same individual. Although, once thought to be experimental, autotransplantation has achieved high success rates and is an excellent option for tooth replacement. Although the indications for tooth autotransplantation are narrow, careful patient selection coupled with an appropriate technique can lead to exceptionally well esthetic and functional results. This procedure is advantageous since the placement of an implant-supported prosthesis or any other form of prosthetic tooth replacement is not needed. This case report is one such example of a successful tooth autotransplantation. A review of the surgical technique as well as success rates is also discussed. Abstract: A relationship between the duration of orthodontic treatment and the exact position of palatally impacted canines before treatment was studied in panoramic radiographs (OPG). Material and Method: Sixty-seven palatally impacted canines were measured in 54 OPGs prior to treatment. The measured parameters were: vertical distance of the canine crown tip to the occlusal plane, inclination of palatally impacted canines to the vertical reference line and horizontal zones. The treatment time was measured from the open eruption technique of the canine till alignment of the canine. Conclusions: A middle correlation was found between the inclination of impacted maxillary permanent canine to the vertical reference line in OPG and the treatment duration. The greater the inclination, the longer the treatment. The relationship between the distance of the canine cusp from the occlusal plane in OPG and the treatment duration was not proved. Horizontal position of the canine in OPG is a factor that significantly affected the treatment duration. Abstract: Background: we investigated the effect of a single preoperative bolus dose of tranexamic acid (15 mg/kg) on perioperative blood losses and packed cell transfusion requirements in patients scheduled for hemiarthroplasty surgery for fracture neck of femur. Patients and methods: Total of 36 patients taken in study . In 17 pts –(Tranexamic acid group)a single bolus dose of 15 mg/kg tranexamic acid was given preoperatively 15 min before the surgery. Data are collected from previously operated patients(19 pts-control group) where tranexamic acid was not given . We recorded per- and postoperative blood losses and transfusion requirements postoperatively.

Abstract: Background: Postoperative pain is one of the primary targets to relieve as it also affects the clinical outcomes Aim: To evaluate and compare the efficacy of Tramadol & Lignocaine during post operative period in terms of analgesia and adverse effect profile in minor Surgeries. Materials and methods: we conducted a prospective, randomized, single blind, parallel group clinical study to compare the local anesthetic effects of Tramadol to Lignocaine in patients who had undergone minor surgeries. The patients were randomly divided into 2 groups (L and T groups) in ASA physical status. Both the groups were administered by subcutaneous route for swellings less than 5cms in size. Swellings selected were Lipoma, sebaceous cyst and dermoid cyst. L group (n=30) patients were administered 1mg/kg Lignocaine and T-group (n=30) patients were administered 2mg/kg Tramadol. The intensity of post operative pain was assessed by using Visual analog scale at 15min, 30min, 1hour, 2hours, 4hours and 6hours. The results were compared and analyzed statistically.

Abstract: Depression, according to WHO predicted to be ranked second only after ischemic heart disease by the year 2020. To make an accurate diagnosis of major depression in the setting of chronic pain is very difficult. Physical symptoms are common in depression, and, in fact, vague aches and pain are often the presenting symptoms of depression. These symptoms include chronic joint pain, limb pain, back pain, gastrointestinal problems, tiredness, sleep disturbances, psychomotor activity changes, and appetite changes. A high percentage of patients with depression who seek treatment in a primary care setting report only physical symptoms, which can make depression very difficult to diagnose. Neurotransmitters level also shows deep connection between pain and depression; as both are mediated by serotonin and norepinephrine, the monoamines. Remission largely depends upon recovery from all the pain symptoms otherwise it may be a cause of relapse.

Abstract: Jejunal diverticular disease is a rare disease and gets complicated even more rarely. Perforation being the least commonly reported complication.We report a case of a 60 year female presenting with crampy abdominal pain since two days and five episodes of vomiting. Abdominal scout x-ray revealed gas under diaphragm and few air-fluid levels. On laparotomy multiple jejunal diverticulae with a diverticular perforation and fecal contamination was present. Peritoneal lavage and resection of diseased segment followed by primary anastomosis was done. Post operative period being uneventful, patient recovered and discharged. Jejunal diverticulosis in the elderly can lead to significant morbidity and mortality and so prompt diagnosis and treatment necessary.

Abstract: Objective: Liver is a potential site for blood borne metastasis for many malignancies of human body, besides giving rise to a primary tumor itself. Often, hepatic metastasis can be the only mode of presentation in certain tumors that prompt us to search for the primary sites. Our study aims at proving that other than gastrointestinal malignancies, specially adenocarcinoma of pancreas, stomach, large gut, gall bladder etc, which are generally known to be notorious for hepatic metastasis, liver can also be a site of metastasis for malignancies from a spectrum of very unusual primary sites. Also through our study we can establish FNAC as a simple, safe, quick and cost effective tool in diagnosing metastatic hepatic lesions.

Abstract: Airtraq is a novel optical laryngoscope which provides glottis display without any deviation in the normal position of the oral, pharyngeal or the tracheal axes. Glottis and the surrounding structures are visualised with the help of the display lens, and under direct view, tracheal tube is introduced between the vocal cords. In patients having restricted neck movements or limited mouth opening (provided that it is greater than 3 cm) Airtraq offers the advantage of a better display. We had a 43 year old male, ASA-I patient with restricted mouth opening of approximately 3.1 cm (21/2 fingers), who was posted for elective open cholecystectomy. We considered the case as an anticipated difficult intubation and decided to use Airtraq (size 3 Regular blue)for intubation. Unfortunately not only the intubation was unsuccessful with Airtraq but also patient had soft palate tear, which required suturing. An attempt with Airtraq was failed due to which a bougie guided intubation was done using laryngoscope.

Abstract:One of the major complications of long term type 2 diabetes is nephropathy.An insertion/deletion
(I/D) polymorphism of the gene encoding angiotensin-I converting enzyme (ACE) is shown to have association
with diabetic nephropathy. The aim of this case control study was to investigate the possible role of ACE gene in
the pathogenesis of nephropathy in patients with diabetes mellitus. The study included 196 subjects (145 T2DM
and 51 normal control) T2DM were classified into 2 groups: 97 diagnosed with DN, and 48 diabetic without
nephropathy as +ve control. randomly selected was conducted to assess the association of SNP ACE gene
polymorphism with diabetic nephropathy in Iraqi population. Blood samples from subjects and controls were
analyzed to investigate the ACE(I/D) genotypes. The homozygous genotype (DD) was significantly (OR=2.73,
CI 95% 1.1- 7.3, P= 0.04) increased the risk of DN two folds with respect to those of the wild type (II) after
adjustment for age, sex and BMI. Also significant variation was obtained when the analysis was carried out
without adjustment(OR=3.95, CI 95% 1.7- 9.2, P= 0.001). Similarly the ID genotype significantly (OR = 3.68,
CI 95%=1.5- 8.7, P= 0.03) raised the risk of DNA by three folds. dominant and recessive models highlighted
significant (P= 0.003) association of dominant model with the risk of DN which raised by three folds. the minor
allele frequency (D) was significantly higher (P=0.04) in DN when compared with that of the normal control
group.

Abstract: Being an edentulous is it a kind of disability in which the affected subject is unable to masticate the food properly which leads to a number of problems related to digestion as well as the health is concerned. Edentulous people are unable to enjoy the food as dentate people and with passing of time they were reluctant to take food properly which leads to malnutrition and other health related issues. Edentulous people lack proper esthetics, having difficulty in speech and so on. Complete denture treatment is the most common and most extensively used treatment for replacing missing tooth across the globe. Complete denture includes the maxillary and mandibular denture replacing the entire tooth. Denture satisfaction is very much of concern to the patient as well the treating clinician as not only involves mastication, it concern speech and esthetics as well to the patient and peers. It is important to know why some patients have more difficulty than others in wearing the denture. The fabrication of denture is play vital role but it also includes medical and psychological well being of the patient. Abstract: Crown fractures of permanent dentition are the most frequent type of dental injury. Ninety-two percent of traumatic injuries results in fracture of maxillary permanent central incisors because it's protrusive and anterior positioning. The young permanent maxillary central incisor root canal chamber is large and tapered and trauma to these type teeth results in wide variety of malformations. The present case report describes the management of mutilated young maxillary right central incisor with Ellis Class IV fracture by using custom modified fiber post technique.

Abstract: The masticatory efficiency of complete denture, implanted supported, root supported and attachment retained overdenture, was checked by the electromyography, which would help in gaining information about the neurophysiologic mechanisms regulating the complex masticatory action and deciding the best treatment for the patients. The study was divided into two groups. In group one, there were five patients, these patients were first given complete denture and then given two implant supported overdenture. In group two, there were five patients, these patients were first given root supported overdenture and then given root supported attachment retained overdenture. Their masticatory efficiency was evaluated during clenching and mastication of masseter and temporalis muscles.

Abstract: Oral health education is most widely used in prevention of dental diseases. However, there is uncertainty about the value of these interventions. The primary objective of this review was to evaluate the effectiveness of oral health education among school children. Methods: The following databases were selected for identification of studies. The Cochrane central register of controlled trials, PubMed, LILACS / BBO, MEDLINE, SCIENCE DIRECT. Studies of at least 3 months duration with clinical evaluation like plaque index, gingival index and DMFT/DMFS index, studies with pre- to post - test design were included for this review. Standardized mean difference is calculated for difference between intervention and control group in plaque score, gingival index scores and DMFT Index values. The meta-analyses was conducted using a random – effects model.
