Country | : | India |
Authors | : | Dr. S. Sivakumar M.D || Dr. N. Kirubanand M.D || Dr. R. Kannan D.M. |
Abstract: Bee stings are commonly encountered worldwide. Various manifestations after bee sting have been described1. Local reactions are common. Unusually, manifestations like vomiting, diarrhoea, dyspnoea, generalised oedema, acute renal failure, hypotension and collapse may occur. Rarely vasculitis, serum sickness, neuritis and encephalitis have been described which generally develop days to weeks after a sting. Acute coronary syndromes after hymenoptera stings and other environmental exposures are referred to as the Kounis syndrome or allergic myocardial ischaemia and infarction. The three variants of Kounis syndrome...........

Title | : | Normally Functioning Tilting Disc Mechanical Mitral and Aortic Valve Prosthesis without Anticoagulant Therapy - An Unusual Case |
Country | : | India |
Authors | : | Dr Shaikh Mohammed Husain DNB Gen Surgery || Dr Vivek Rawat M.S || Dr Rajkumar Yadav.Mch |
Abstract: Anticoagulation after mechanical heart valve replacement is indispensable to prevent valve thrombosis and it's consequences. At the same time, excess dosing leads to complications related to bleeding. One of our patients who underwent double valve replacement for infective endocarditis lost to the follow up after 5 months of surgery then he turned back after 11 years with history of discontinuing anticoagulants. International normalised ratio (INR) was 0.97. Further evaluation revealed normally functioning mechanical mitral and aortic valve (Medtronic tilting disc).

Abstract: Background: Squamous cell carcinoma is commonest histologic variant of carcinoma of esophagus (ESCC). It is predominant in developing countries and usually has a poor prognosis, because of its advanced stage at the time of diagnosis. Understanding the prognostic factors helps in selection of the most appropriate surgical approach in order to select patient for target directed therapies. HER-2/neu (C-erbB-2), member of human epidermal growth factor receptors (EGFR) is important, as it plays a crucial role in therapeutic implications of esophageal carcinoma. Considering the importance of c-erbB-2 expression and its prognostic significance in esophageal cancer, this study was undertaken.............

Abstract: Human Immunodeficiency Virus (HIV) infection and Tuberculosis (TB) increase the rate of disease progression of each other, thereby reducing the survival time of the patients. TB disease is considered a preventable, detectable and curable disease that requires wider public- private partnerships. Causes of mortality include late TB and/or HIV diagnosis, late HAART initiation, overlapping drug toxicities or comorbidities, drug-drug interactions between rifamycin and antiretroviral drugs, and immune reconstitution syndrome of TB after ART initiation etc............

Title | : | Comparison of Hemodynamic, Emergence and Recov Ery Characteristics of Sevoflurane with Desflurane in General Anesthesia |
Country | : | India |
Authors | : | Dr.Vairavarajanchandrasekaran || Dr. Lakshmi Sudha |
Abstract: General anesthesia is a dynamic balance between the level of hypnosis, analgesia, and stimulation. It is usually defined as a triad of amnesia, analgesia, and muscle relaxation. Inhaled volatile anaesthetics remain the most widely used drugs for maintenance of general anesthesia because of their predictable intraoperative and recovery characteristics. Management of haemodynamic stability and early recovery is the most important part of a standardized balanced technique. The aim of this study is to prospectively compare the hemodynamic, emergence and recovery characteristics of sevoflurane with that of desflurane in general anesthesia............

Abstract: Inhalation of BiomassFumes over a long time leads to proliferation and fibrotic changes in Lungs. Early recognition of this damage provide, an important clue to insure good health. Spirometry is an invaluable screening test to identify patient with air flow obstruction. The study has been conducted as 100 Rural and 100 Urban Punjabi females using Biomass fuel and LPG respectively between 20-50 years of age. The ventilatory tests like PEFR, FEV measured by computerized spirometer (Med-Spiror) showeda highly significant decline in Rural women,While in rest of the parameters there is statistically non-significant decrease in rural women...........

Title | : | Cystatin C Based Evaluation of Renal Function in Hypertensive Patients in Ubth, Benin City |
Country | : | Nigeria |
Authors | : | Alaje Abiodun K || Idogun Sylvester E || Idemudia Joseph O |
Abstract: Background: Hypertension is a recognized cause of chronic kidney disease. Hypertension in blacks is most important because it is volume dependent and its ensuing renal complication. The widely accepted marker to assess renal function is creatinine and its estimated glomerular filtration rate. However, this is limited in assessing early renal impairment. This necessitate a need to evaluate other renal markers such as cystatin C. Objective: To measure and compare serum cystatin C and creatinine for early renal impairment among hypertensive patients on treatment.........

Abstract: Percutaneous nephrolithotomy (PCNL) is now considered as the standard treatment for large and complex renal stones. The outcomes of PCNL can be interpreted in terms of success and complication rates. Recent attempts to report complication rates of PCNL have focused on stratifying them by severity by use of the Clavien Dindo grading system. The Guy's Stone Score (GSS) used to classify the burden of stones. This was a prospective study conducted by the department of Urology, NIMS hospital, Jaipur. In study of 508 PCNL cases, overall stone free status achieved in 88..............

Title | : | Dexmedetomidine as an adjuvant for Hypotensive Anaesthesia during Functional Endoscopic Sinus Surgery (FESS) |
Country | : | India |
Authors | : | P. Gupta || R.Choudhary || T. Ojha || D.Jethava |
Abstract: Introduction:FESS (Functional Endoscopic Sinus Surgery) is done via endoscope and the area is highly vascular thus it becomes important to minimize bleeding. Hence we require hypotensive anesthesia. Various drugs have been used for the purpose: nitroglycerine (NTG), sodium nitroprusside(SNP),propofol,beta blockers, calcium channel blockers, higher concentrations of inhalational anesthetics etc. Since all these drugs have certain limitations there was a search for safer and more effective drug .Dexmedetomidine, a newer alphaadrenoceptor agonist,fulfills this requirement since it is short acting, has no residual effects, produces
sedation and analgesia and reduces mean arterial pressure thereby reducing intraoperative blood loss............

Title | : | Relationship Of Platelet Distribution Width And White Blood Cell Count On Admission With St-Segment Resolution In Patients With St-Elevtion Myocardial Infarction Thrombolysed With Streptokinase" |
Country | : | India |
Authors | : | Dr.Surendran.R || Dr.Ganesamoorthy, K M.D |
Abstract: There is an alarming rise in the incidence and mortality of myocardial infarction. In the context of management of myocardial infarction there are few strategies and indicators to risk stratify patients. Taking the hypothesis that platelets produced during an acute thrombotic event is larger and aggressive we studied the relationship between platelet distribution width (which is a more specific marker of platelet size variation) and the success of thrombolysis in patients with STEMI treated with streptokinase as evidenced by ST segment resolution in ECG after thrombolysis (an ST segment resolution of more than 50% after 60 minutes of thrombolysis is taken as successful thrombolysis).......
