Abstract: The most common cause of a breast mass in young females is fibroadenoma, accounting for approximately 75% of all breast lesions in young females [1]. However only 0.5-2% of all cases of fibroadenomas can be classified as giant fibroadenomas. Usually one or two fibroadenomas occur in a patient, but bilateral lesions or even up to four in a single breast is a relatively common feature. Ultrasound is particularly useful in evaluation of fibroadenomas since young women commonly have dense breast tissue, rendering mammography more difficult. The postoperative recurrence of fibroadenomas in patient perhaps was due to the constant presence of the predisposing factors and should not be considered a relapse. Material and methods:A study of 100 cases of breast lumps were studied for the evaluation of unusual types of fibroadenomas.

Abstract: Background: Dacryocystitis is the inflammation of the lacrimal sac due to nasolacrimal duct obstruction. Chronic dacryocystitis is a constant threat to cornea and orbital soft tissue. Surgery is the mainstay for its treatment. The gold standard procedure of choice for the treatment of epiphora is Dacryocystorhinostomy (DCR). This study proposes to evaluate and compare the surgical outcome of External DCR and Transcanalicular laser DCR (TC- DCR). Methods: An Open Level Randomized Clinical Trial study was conducted from December 2013 to May 2015. 80 diagnosed cases of chronic dacryocystitis were randomized into two groups, External DCR and TC- DCR groups. Each of them included 40 patients.

Abstract: The propyl derivative of the pipecoloxylidides was less toxic than the butyl derivative (bupivacaine). Further work revealed that the nerve blocking properties of the R and S enantiomers were similar but that the S-enantiomer was less cardiotoxic. Thus the S enantiomer of the propyl derivative (Ropivacaine) was chosen for further development.Ropivacaine,structurally resembling bupivacaine, with a propyl group on the piperidine nitrogen atom of the molecule is a relatively new amino-amide anaesthetic agent, similar in chemical structure to bupivacaine has been recently launched in India, for clinical evaluation having various advantages like early onset and shorter duration of action and having lesser cardio toxicity as compared to bupivacaine. The drug ropivacaine, relieves the psychological distress of being immobile for a longer period of time after lower abdominal surgeries.In view of the above context the present study was undertaken for comparison of isobaric ropivacaine and isobaric bupivacaine to determine clinical efficacy of ropivacaine.

Title | : | None Resolving Appendicular Mass: A Rare Case Report |
Country | : | India |
Authors | : | Niharika M || Prabhakara Rao Y || D.Poornima || Satya Navamani |
Abstract: At present, the treatment of choice for uncomplicated acute appendicitis in adults continues to be surgical. The inflammation in acute appendicitis may sometimes be enclosed by the patient's own defense mechanisms, by the formation of an inflammatory phlegmon or a circumscribed abscess. The management of these patients is controversial. Immediate appendectomy may be technically demanding. The exploration often ends up in an ileocecal resection or a right-sided hemicolectomy. Recently, the conditions for conservative management of these patients have changed due to the development of computed tomography and ultrasound, which has improved the diagnosis of enclosed inflammation and made drainage of intra-abdominal abscesses easier.

Abstract: Juvenile nasopharyngeal angiofibroma is a locally aggressive benign vascular neoplasm, composed of vasogenic and myofibroblastic elements, accounts for 0.05–0.5% of all the head and neck neoplasms. There are very few case reports of nasopharyngeal angiofibroma involving the oral cavity; we report a case involving both the maxilla and mandible in a 12-year-old patient who reported with a large firm swelling on left side of face with recurrent epistaxis.The contrast CT showed a large mutilobulated soft tissue mass epicentered in the left sinonasal cavity with extention into left parapharengeal space postero-laterally, into the middle cranial fossa, sphenoid sinus and cavernous sinus superiorly, posterior nasopharynx posteriorly, oral cavity inferiorly, infratemporal and masticator space laterally and also submandibular space. It was supplied by the right external carotid artery. Open surgical excision of the mass was done. The diagnosis at an early stage is important because it is associated with high risk of morbidity, but advances in imaging, and surgical methods of treatment have changed the outcome of this rare disease.

Title | : | Study of Thyroid Dysfunction in Patients with Rheumatoid Arthritis |
Country | : | India |
Authors | : | Kiran Tandia (MD) || Shikha Agrawal (MD) |
Abstract: The relationship between thyroid disease and rheumatic disorders has been the subject of considerable debate. Thyroid dysfunction and/or autoimmune thyroid disease were observed in patients with rheumatoid arthritis (RA), which could be attributed to the natural feature of autoimmune diseases and their tendency to overlap. Consideration of the fact that autoimmunity plays a role in the pathogenesis of both RA and hypothyroidism has raised the need to study the frequency of thyroid dysfunction and thyroid antibodies in RA patients and their relation to disease activity.

Title | : | Attenuation of Intra-Operative Pressure Response to Pneumoperitoneum in Patients Undergoing Laparoscopic Cholecystectomy – A Comparative Study between Dexmedetomidine and Esmolol |
Country | : | India |
Authors | : | Nirav Kotak || Nilesh Patil || Yogesh Rathod |
Abstract: Dexmedetomidine is a selective α2-adrenergic receptor agonist which is known to produce sedation, analgesia, and also has sympatholytic, anaesthetic sparing and haemodynamic stabilising properties without significant respiratory depression.Its sympatholytic effect decreases mean arterial pressure and heart rate by reducing norepinephrine release and hence improves haemodynamic stability during laparoscopy Esmolol is an ultra-short-acting β1-adrenoceptor antagonist without any partial agonistic action or local anaesthetic action which is known to produce haemodynamic stability during laryngoscopy, intubation and pneumoperitoneum .

Title | : | Attitude of Yemeni Dental Practitioners toward some Technical Aspects of Endodontic Treatment Procedures |
Country | : | Dhamar, Yemen |
Authors | : | Ahmed A. Madfa || Fadhel A. Al‑Sanabani || Hamzah A. Al-washali |
Abstract: This study was conducted on the Yemeni dental practitioners to gather data on the current opinions of the dental practitioners regarding some technical aspects of routine endodontic practice. Dental practitioners were asked to choose only answer that best fitted their clinical performance. The Chi-square test was used to compare proportions among groups and the significance threshold was set at p < 0.05. Of the respondents, only 7.1% used rubber dam isolations during endodontic treatments compared to other isolation method. A majority of respondents (37.1%) used radiographic method while whose used both electronic and radiographic method were 21.4%. Sodium hypochlorite was the most popular choice as a root canal irrigant (60.9%), while 12.9% used chlorhexidine. The cold lateral compaction of gutta-percha in conjunction with a root canal sealer was used by 85.7% of the respondents, while 12.9% used a single cone technique. Zinc-oxide eugenol root canal sealer (68.6%) was most frequently chosen, followed by N2 (31.4%). A majority of the respondents agreed that want to improve their clinical practice (92.9%) and agreed to carry out continues education programmes in endodontics (95.7%).This study shows that majority of respondents still used conventional materials and techniques for filling root canal system.

Authors | : | Dr. Rashmi S || Dr. Arthi Rasquinha || Dr. Akshay K.M. || Dr.Vidya Hegde || Dr. Rashmi Jain || Dr. Anupama B. |
Abstract: Purpose: To assess the difficulties faced by spectacle wearing urban youth, in day to day life. Methods: This is a cross sectional study where the participants filled up a semi structured 17 item questionnaires.
Results: A total of 118 participants took part in the survey (40.7% males and 59.3% females). Mean age was 20.76±1.8 years. The most common difficulties faced were fogging of the glasses while drinking hot beverages and while getting out of AC car or room (72%), scratches on glasses (67.6%), blurring during rains (66%), need for careful handling (64.5%), difficulty to watch 3D movie (62.7%), issue regarding cost of frames and glasses (61%), feeling of dependency on glasses (61%) and inability to wear sunglasses ( 56%). Some other problems were glare, improper fit, difficulty to take part in sports or recreational activities, perceived personality by self and others. There was a statistically significant association between gender and cost of frames and glasses (Females, p 0.05) and also between gender and inability to wear sunglasses (Males, p< 0.05)
Conclusions: In spite of the awareness and access to facilities, the urban youth were bothered by certain spectacle related challenges. The treating ophthalmologist needs to address these issues and guide them based on their needs.

Abstract: PityriasisLichenoidesChronica(PLC) ,PityriasisLichenoides Et VarioliformisAcuta(PLEVA), Febrile UlceronecroticMucha-Habermann Disease(FUMHD) all represent clinicopathological spectrum of same disease entity. FUMHD is a rare febrile variant of PLEVA characterised by necrotic cutaneous ulcerations associated with high fever and systemic manifestations. Hereby we report a 11 year old male, who initially presented with lesions of PLC which healed with oral erythromycin for 8 weeks but in 3 months he again presented with generalised ulceronecrotic lesions on skin, continuous high grade fever and abdominal pain suggestive of Mucha-Habermann Syndrome. He responded only to systemic corticosteroids. Disease being a clinical spectrum, it is important to follow up the patients presenting with PLC and having a watchful suspicion as it can turn into potentially fatal MHD.

Title | : | Improved Detection and Localization of Apical Lytic Lesions in Previously Obturated Teeth Using Cone Beam Computed Tomography as Compared to Conventional Radiography |
Country | : | USA |
Authors | : | Michael Elliott DMD, MS || Madhu K. Nair DMD, MS, Lic. Odont., PhD || Uma Nair DMD, MDS |
Abstract: Purpose: To determine the diagnostic ability of Cone Beam Computed Tomography (CBCT) to detect and localize apical pathoses as compared to two-dimensional conventional direct digital intraoral radiographs (DDR), in order to accurately determine the location and extent of these lesionsto improve treatment outcomes.
Methods: Previously obtained CBCT and DDR data from 45 patients with a diagnosis of failed endodontic therapy were included. Images were analyzed by three calibrated observers for the presence of periapical radiolucencies, number and location of roots involved, and any cortical erosion/perforation resulting from the lytic lesion. The readings were repeated after an interval of two weeks.
Results: Significantly more number of roots (25%) involved with periapical pathoses were detected using CBCT. Localization of cortical erosions/perforations was possible only with CBCT. Validation of imaging findings was effected using direct visualization of the regions of interest at the time of surgery.
Conclusions: CBCT images were significantly superior to DDR for detection of apical pathoses and localization of erosion/perforation of cortices with respect to roots of teeth.Results illustrate the advantages of three-dimensional imaging for localization of pathoses in diagnostically challenging cases with possible implications on treatment planning.Accurate localization of these lesions and a higher and accurate detection rate may result in better outcomes in re-treatment cases. More studies are needed to assess the time required for full healing post endodontic treatment using the additional information gleaned from CBCT data.

Abstract: Amelogenesis Imperfecta is a developmental disorder of genomic origin associated with structural enamel formation. Although AI is considered as a single disease entity, it actually represents a group of heterogenous conditions, with diverse structural defects of enamel resulting in a range of clinical phenotypes. It is characterized by clinical and genetic heterogenecity in the absence of systemic abnormalities or diseases. This case report presents with a case of AI as an example of a hereditary disorder.

Title | : | Review of Parameters Used To Assess Hearing Improvement in Tympanoplasty |
Country | : | India |
Authors | : | Sachin Gupta || Parmod Kalsotra || Shelly Sehgal || Navneet Gupta |
Abstract: Restoration of hearing is associated with resocialization of those affected and thus forms an important aspect of rehabilitation of individuals suffering from chronic suppurative otitis media. The surgeons with time have become more interested in returning back the hearing along with treating infection in the middle ear cleft. The zeal to restore hearing has lead to the evolution of newer surgical techniques as well as improved indicators of surgical outcome. However, even after six decades of introduction of tympanoplasty, the goal of achieving stable and long term hearing results continues to be ambiguous. Different authors in the past have used different methods to compare the postoperative hearing results. Lack of universal criteria and the shortcomings of the different variables used in different populations has been a major obstacle in reporting of improved hearing results after tympanoplasty. Postoperative air conduction threshold, average hearing gain, post-operative air-bone gap and air bone gap closure are few of the parameters commonly used to assess the hearing outcome of tympanoplasty. The article provides a comprehensive review of the parameters when used alone or in combination with others and the results in terms of success obtained in light of these variables.

Title | : | A Study On Awareness Regarding HIV/AIDS Among Attendee Of Integrated Counselling And Testing Centre Of RIMS, Ranchi, Jharkhand |
Country | : | India |
Authors | : | Dr Vivek Kashyap || Dr Luguram Tudu || Dr Shalini sunderam || Dr Vidyasagar |
Abstract: Background: Acquired immunodeficiency syndrome (AIDS) is pandemic worldwide and one of the most dreaded entities that modern medicine has ever had to tackle. Since there is no cure of this disease. Awareness about transmission and protection from HIV/AIDS reduces disease prevalence. Aim and
Objectives: (1) Socio demographic profile of attendee of ICTC. (2) To assess the awareness on HIV/AIDS. Material and Methods: A cross sectional, descriptive, institutional based study was conducted at ICTC of RIMS, Ranchi, and Jharkhand between march 2015-may 2015. Total 116 subjects were included in the study. Pre tested, semi structured questionnaire was used for data collection. Statistical analysis: Data entry was done in MS excel and analyzed in SPSS software version 20.0.
Results: Out of 116 attendee majority were male (68.1%), unmarried (56%), Hindu (56.9%), non tribal (65.5%), graduate (40.5%) and belonged to age group of 26-35 years of age (42.2%). Less than half (48.3%) first heard about HIV/AIDS from newspaper/television. About 75% of attendee responded that unprotected sex is the main cause of HIV transmission and 69.8% people stated that disease can be protected by practicing safe sex by using condom.
Conclusion: ICTC is the place for testing, diagnosis, referral for treatment, care and high risk behavioural changes. About two third of respondent were aware of transmission and protection from HIV/AIDS in this study. However intense IEC activities regarding spread and prevention from HIV/AIDS and behavioural changes is necessary to reduce further spread of the disease.
