Abstract: Introduction :Subarachnoid block using local anesthetics is choice of anesthesiafor lower limb surgeries .We conducted study on 75 patients 25 each group of 1,2 and 3ml of 0.5% heavy bupivacaine(8% glucose) for spinal anesthesia in lower limb surgeries. We compared effect of spinal block between dependent limb and non dependent limb after giving spinal anesthesia in lateral decubitious position. Aim: To study and compare dependent with nondependent limb fortime taken for onset, highest sensory dermatomal level and duration of recovery of subarachnoid block in lateral decubitious position using different volumes of 0.5% heavy bupivacaine in patients posted for lower limb surgeries............

Abstract: Carcinoma of the stomach is still remained among the most common cause of cancer death worldwide. In an effort to understand interaction between force of Intercellular adhesion and its disruption by propagating cancer cells we have selected 46 caresses of Gastric adenocarcinomas. As E-cadherin (ECD) is the strongest intercellular adhesion molecule in nonmalignant Gastric mucosal epithelial cell and Autocrine motility Factor (AMF) is an established propagator of its malignant counterpart, we have studied simultaneous expression of ECD and AMFR (Autocrine motility Factor Receptor).With aid of immunohistochemical technique, strong or weak expression of ECD and AMFR was determined according to predetermined parameters .Normal Gastric mucosal cell (the control cases) strongly express ECD whereas reverse was true with AFMR. Abstract: Objective Resuscitation in major burn injury-experience in tertiary care centre for better outcome. The study is a Observational and Prospective study of 100 cases of Thermal Burn ,admitted in Subharti Medical College and Hospital, Meerut, during the period from 2015 to 2017.A total of 100 patients of acute thermal burn presenting in the first 12 hrs were included in the study. Abstract: Objective: To study clinical profile and etiology of liver ,to determine the role of ultrasonography in the management of liver abscess and the outcome associated with different treatment strategies of liver abscess. Materials and methods:All patients with liver abscess 2015 to 2018 were included in this study.50 patients with Liver abscess who reported to various departments of Subharti medical College, Meerut, were included in the study. The patients were treated with antibiotic alone for abscess size<5 cm, patients were treated with percutaneous needle aspiration for abscess size 5-10cm and for abscess more than 10cm patients were treated with pigtail catheterization. Those patients who presented with rupture liver abscess were treated by surgical approach by laparotomy. PNA was repeated every third day if the cavity size had not declined to 50% of the original for up to twice times on repeat USG. Abstract: Objectives: To study the efficacy of intralesional triamcinolone acetonide injection in the management of chalazion. Materials And Method: A prospective clinical study on patients with chalazion less than 5mm in size attending the OPD of department of ophthalmology in a tertiary centre. RESULT:Patient with chalazia of size<5mm responded well to the treatment. CONCLUSION:We conclude from the present study that intralesional triamcinolone acetonide injection is a simple, safe and effective alternative to surgery in the treatment of small chalazia.

Abstract: Since time immemorial, the diagnosis of haematological disorders were mainly done by using bone marrow aspiration. The available statistics show that the diagnosis of haematological disorders taking complete clinical history, careful physical examination, complete haemogram and Bone marrow examination by either aspiration (or) biopsy. Even though aspiration is diagnostic jamshidi introduced a biopsy needle that was easier to use and which provided similar intact biopsy specimens. The biopsy is more superior than aspiration.

Abstract: Introduction: Acute pancreatitis being a frequent casuality in the surgical department with griever consequences need an early diagnosis and treatment. In a developing country it is utmost important to diagnose such case and refer it to the tertiary care hospital for further evaluation and treatment to decrease the mortality rate. With this background the present study was carried out to examine the possibility of an easily conclusive test for acute pancreatitis which can be performed at the side laboratory of any hospital. Acute pancreatitis due to various etiology was diagnosed with the classical test, depedent on amylase and lipase activity. In this study the effect of protease fraction of pancreatic enzyme (mainly trypsinogen) which produce free amino acids and small peptides (2-8 amino acids prod, protease activated product PAP) was evaluated. During a phase of acute pancreatitis these fractions are expected to reflect the effect of circulating protease activity either as a relatively direct observable parameter or as an index derived from computation of some functional test.........

Abstract: Recurrentoral ulcers have been estimated to affect more than 10 million people per year in India.Though it is a much common condition which presents as recurrentsmall, ovoidoral ulcers with circumscribed margins, erythematous halos and yellow floors, still etiology remains a debatable one. Recurrent aphthous ulcers may be a manifestation of a disorder rather than a separate entity (1). So a better drug with maximal efficacy andminimal side effect is the need of the hour and so we conducted a prospective study on efficacy of Rebamipide in recurrent oral aphthous stomatitis.Rebamipide is already marketed as a gastro protective agent. Our study conducted in a tertiary care centre has approved the efficacy of Rebamipide in recurrent aphthous stomatitis, which can be marketed as a promising drug for the same.

Abstract: Introduction: The injection of local anesthesia with adrenaline in oral surgical procedures is associated with the metabolic changes in blood glucose level.The present paper aims to study the changes in blood glucose level after injection of lignocaine with adrenaline Materials and Methods: The study was carried out on 30 healthy subjects who needed minor oral surgical procedure under local anesthesia.The patients were excluded for any blood sugar or other metabolic and endocrine disorders.The procedure was carried out with with 2ml of lignocaine with adrenaline and the blood glucose was measured pre and post operatively after 10 minutes. Abstract: Introduction: Prednisone (PDN) is the most commonly used first-line treatment for primary immune thrombocytopenia (ITP) in children. Recently, a short-course high-dose dexamethasone (HD-DXM) therapy has been reported to be efficacious in childhood primary ITP. However, the choice of best corticosteroid as an initial therapy in primary ITP in children is still a matter of debate. Methods: This single-blind prospective randomized clinical trial was conducted at Pediatrics Department, NRS Medical College and Hospital, Kolkata, a tertiary care teaching institute in the state of West Bengal from the month of July 2014 to June 2017 to compare the efficacy and safety of HD-DXM and conventional PDN as first-line strategies for newly diagnosed primary ITP in children. Patients enrolled were randomized to receive DXM 20 mg/m2/day for 4 days (n=49) or PDN 2.0 mg/kg/day for 10 day and then tapered.........

Abstract: Freshly extracted 80 intact, non-carious single rooted human maxillary central incisors that were extracted for periodontal reasons within six months period of start of the study were collected from the Department of Oral and Maxillofacial Surgery. The root canals were instrumented using a step-back technique. Obturation was done with cold lateral compaction method using Gutta-Percha. Abstract: The cranial base area of the craniofacial complex has long been of interest to orthodontists and craniofacial anthropologists. The cranial base provides support for the brain and adaptation during growth between the developing neurocranium and viscerocranium. The aims of this investigation was to assess if there is any evidence that the cranial base angle predisposes the jaw base relationship in a Dravidian population, and to gain data specific for this population with particular reference to the angular and linear cranial base morphology. Materials And Methods: The study involved 105 subjects (male:52 ; female:53 : age: 18± 5 SD years) from Chennai who were classified into 3 sagittal discrepancy group on the basis of their ANB angle.A cephalometric analysis of the angular and linear measurements of the cranial and jaw bases was carried out. The morphological characteristics of the cranial and jaw bases in the three groups were compared and assessments were made as to whether a relationship existed between the cranial base and the jaw base discrepancy.............

Abstract: Oral squamous cell carcinoma is the most common malignancy of the oral cavity. Although oral cancers are usually visible and accessible, delays in diagnosis are not uncommon. The aim of the present study was to characterize diagnostic delay of oral cancers in a cohort of Bulgarian patients. The mean total delay of oral cancer diagnosis was found to be 7.3 months (median 4 months) in 288 patients included in the study. Abstract: Tramadol and fentanyl both have analgesic and sedative properties when used as adjuvant in regional anaesthesia. Analgesic and anaesthetic requirement get reduced by use of these adjuvant. They have stable hemodynamics and enhanced sympathoadrenal stability. Pain relief is essential for optimal control of patients in the intraoperative and post operative period. Epidural technique has opened a new horizon in modern anaesthesia for pain relief. Benefits of good quality epidural analgesia include- early ambulation, faster postoperative anaesthetic recovery, better post operative analgesia ,earlier return of bowel function, decreased incidence of thromboembolic phenomenon. Abstract: Dental abfraction is non-carious cervical lesion resulting from flexure forces. As the tooth flex under occlusal load, stresses are transmitted to the cervical area, causing the cervical enamel to fracture and dislodge, and with increased flexure, a v- shaped notch develops. The pathogenesis is related to the occlusal stress that causes non-carious lesion in the enamel and dentin at the cervical region. Clinically, it presents as notches in the tooth structure near or even under the gum line usually on the cheek side of tooth. The restoration treatment is made using an aesthetic adhesive material, but we must take into account the causal factors and the specific area where the restoration is done. This report present a case of abfraction which was successfully treated by Composite restoration material.

Abstract: Insulinoma is rare pancreatic islet cell neuroendocrine tumor typically sporadic, solitary and benign. We report the case of sporadic insulinoma in 16-year-old girl who presented with chief complaints of loss of consciousness episode daily since 7 month associated with palpitation, tremors, and diaphoresis. Clinical examination and laboratory investigation suggestive of pancreatic insulinoma, then patient admitted for further workup. A fasting test of 48-hour was done and ceased within 3 hours due to occurrence of hypoglycemic symptoms. During the episode, blood was drawn and results showed low blood glucose level and high insulin, pro-insulin and C-peptide levels.........

Abstract: By eliminating future problems like stress concentration and a fulcrum on the pier abutment, longevity of the fixed partial denture can be maintained. An appropriate means of causing a stress breaking action is by use of a precision attachment (tenon-mortise) for adequate dissipation of forces along the long axis of the tooth and not in any other direction. Transfer of shear stresses to the underlying bone is of utmost importance to avoid loosening of the pier abutment over a period of time............

Abstract: Urinary calculi are the third most common problem affecting the urinary tract, followed by urinary tract infections& diseases of the prostate. Objective of this study is to evaluate and compare the efficacy and safety of Pneumatic versus laser lithotripsy in the management of ureteric stones.Stone migration was more common on pneumatic lithotripsy group than the laser group. Duration of surgery was more in laser group than pneumatic group. Both of them achieved complete stone clearance with respect to lower ureteric calculi. For upper ureteric calculi, because of less stone migration in laser group, it resulted in better stone clearance.
